Requirements
- Active compact multi-state LPN/LVN license in the state of residence.
- Active California state LVN license.
- Minimum 5 years of clinical experience as a Licensed Practical Nurse.
- Experience providing patient education and support for chronic condition management.
- Proficiency with AI-assisted clinical tools such as Gemini or Open Evidence.
- Reliable high-speed internet for remote work readiness.
- Ability to navigate and troubleshoot multiple systems simultaneously.
- Strong interpersonal skills to build trust with patients over the phone.
- Familiarity with evidence-based techniques like motivational interviewing and SMART goals.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions.
- Help patients execute personalized care plans through education on behavior modification, nutrition, and self-management.
- Conduct comprehensive assessments of patients' health status, lifestyle behaviors, and readiness to change.
- Educate patients on disease management, medication adherence, and symptom recognition according to clinical protocols.
- Monitor patient progress and adherence using remote monitoring tools and regular check-ins.
- Document patient interactions and follow-up plans in the platform while identifying barriers to care.
- Collaborate in real time with clinicians and support teams using Google Workspace, Slack, and internal platforms.
