Requirements
- Active compact multi-state LPN/LVN license in current state of residence.
- Active California LVN license.
- Minimum 5 years of clinical experience as a Licensed Practical Nurse.
- Remote work readiness with stable high-speed internet.
- Experience in patient education, coaching, and care coordination.
- Comfort using AI-assisted clinical tools (e.g., Gemini, Open Evidence).
- Proficiency in navigating multiple digital systems simultaneously.
- Strong interpersonal communication skills for phone-based patient engagement.
- Ability to work independently in a fast-paced environment.
- Fluent Spanish preferred.
- Experience with chronic condition support preferred.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching for patients managing chronic conditions like diabetes, hypertension, and cardiovascular disease.
- Help patients execute personalized care plans through education on behavior modification, nutrition, physical activity, and self-management.
- Conduct comprehensive assessments of health status, lifestyle behaviors, nutritional habits, and readiness to change.
- Educate patients on disease management, medication adherence, symptom recognition, and prevention strategies.
- Monitor patient progress, treatment adherence, and health outcomes using remote monitoring tools and regular check-ins.
- Document interactions and identify barriers to care to connect patients with necessary resources.
- Collaborate in real time with clinicians and support teams using Google Workspace, Slack, and the Cadence platform.
