Requirements
- At least 3 years of experience in a health education-related capacity.
- At least 1 year of experience working with member populations within a managed care setting.
- Experience in asthma instruction (AE-C certification preferred).
- Experience working with a pediatric population.
- Knowledge of HEDIS and NCQA standards.
- Knowledge of community resources.
- Proficiency in Microsoft Office suite and ability to learn new information systems.
- Ability to build relationships with community-based organizations.
- Strong customer service, critical thinking, problem-solving, and analytical skills.
- Ability to travel (same day or multi-day) up to 80% depending on state-specific needs.
Responsibilities
- Develop, implement, and evaluate health education programs for members and the community.
- Identify, design, and implement health education interventions based on department and contract priorities.
- Conduct population, geographic, and member-specific needs assessments.
- Engage members in health promotion via direct member calls.
- Coordinate member care through call tracking and health education voicemails.
- Review and update health education materials, preventive care guidelines, and incentive packets.
- Represent the organization as a liaison to internal departments, CBOs, social services, and public health agencies.
- Collaborate on quality improvement (QI) planning and projects, including HEDIS activities.
- Provide training and support to new and existing health education team members.
