Requirements

  • Prior experience in case management or a healthcare setting (preferred).
  • Proficiency in motivational interviewing techniques.
  • Strong telephone and verbal communication skills with the ability to navigate conversations effectively.
  • Demonstrated professional written communication skills.
  • Highly organized with strong time management skills and ability to work autonomously.
  • Ability to multitask and adapt to a changing environment.
  • Proficiency in computer technology and ability to quickly learn new platforms.
  • Experience with health IT systems and reports (desirable).
  • Knowledge of community healthcare and social welfare resources (preferred).
  • Understanding of patient rights and ability to maintain confidentiality following HIPAA guidelines.
  • Commitment to scheduled working hours: 8-5PM (PST/CT/EST) Monday-Friday.

Responsibilities

  • Engage members through scheduled calls, chats, and targeted outreach shifts using autodialer tooling.
  • Coordinate care for members, families, and caregivers as identified by the clinical team.
  • Recommend employer-provided benefits and initiate care cases such as Provider Match and claims.
  • Complete administrative tasks including composing member/provider letters, obtaining medical records, and monitoring insurance feeds.
  • Triage callback tickets, referrals, and escalations.
  • Attend team huddles and ongoing healthcare training.
  • Comply with organizational guidelines and HIPAA healthcare laws.
