Requirements
- Prior experience in case management or a healthcare setting (preferred).
- Proficiency in motivational interviewing techniques.
- Strong telephone and verbal communication skills with the ability to navigate conversations effectively.
- Demonstrated professional written communication skills.
- Highly organized with strong time management skills and ability to work autonomously.
- Ability to multitask and adapt to a changing environment.
- Proficiency in computer technology and ability to quickly learn new platforms.
- Experience with health IT systems and reports (desirable).
- Knowledge of community healthcare and social welfare resources (preferred).
- Understanding of patient rights and ability to maintain confidentiality following HIPAA guidelines.
- Commitment to scheduled working hours: 8-5PM (PST/CT/EST) Monday-Friday.
Responsibilities
- Engage members through scheduled calls, chats, and targeted outreach shifts using autodialer tooling.
- Coordinate care for members, families, and caregivers as identified by the clinical team.
- Recommend employer-provided benefits and initiate care cases such as Provider Match and claims.
- Complete administrative tasks including composing member/provider letters, obtaining medical records, and monitoring insurance feeds.
- Triage callback tickets, referrals, and escalations.
- Attend team huddles and ongoing healthcare training.
- Comply with organizational guidelines and HIPAA healthcare laws.
