Requirements
- Registered Nurse (RN) with an active California license.
- 2+ years of home health nursing experience.
- Strong working knowledge of OASIS (OASIS-E preferred).
- Proficiency in ICD-10 home health coding.
- In-depth understanding of CMS regulations and Medicare Conditions of Participation (CoPs).
- Knowledge of state licensure requirements.
- Strong analytical skills with attention to detail.
- Excellent clinical judgment.
- Effective communication and collaboration skills for interdisciplinary teamwork.
Responsibilities
- Review all OASIS assessments prospectively to ensure accuracy, completeness, and regulatory compliance.
- Analyze OASIS variation and alert reports to identify documentation gaps and trends.
- Ensure correct ICD-10 coding and sequencing, including primary diagnoses and co-morbidities.
- Collaborate with home health clinicians to clarify documentation issues and support necessary corrections.
- Review visit utilization to ensure services align with patient condition, care guidelines, and reimbursement requirements.
- Identify potential utilization risks and report findings to the Clinical Supervisor.
- Communicate trends in OASIS data and outcomes to leadership.
- Participate in Quality Improvement (QI), performance improvement, and Corporate Compliance activities.
- Assist with additional chart audits and clinical documentation reviews as needed.
