Requirements

  • Nurse Practitioner with Full Practice Authority in good standing with active US licensure.
  • OR Physician (MD/DO) board-certified in Geriatrics, Lifestyle Medicine, or Family Practice.
  • Training or board certification in lifestyle medicine (DipACLM / DipIBLM) or active pursuit of certification.
  • Deeply versed in lifestyle medicine principles.
  • Experience caring for older adults.
  • Comfort practicing via telehealth in an ambiguous, building-phase environment.
  • Ability to license in multiple states.
  • Strong coaching and motivational skills for patient behavior change.
  • Mission-driven and collaborative approach to clinical practice.

Responsibilities

  • Partner with older adults to assess health goals and build personalized, evidence-based lifestyle care plans.
  • Coach patients through sustainable behavior change including nutrition, movement, sleep, stress management, and social connection.
  • Apply the 4 M's of Age-Friendly care (What Matters, Medication, Mentation, Mobility) to all patient plans.
  • Help shape the clinical model, including intake protocols, outcome measures, and documentation standards.
  • Collaborate with the behavioral health team to support whole-person care.
  • Coordinate with patients' existing primary care providers to avoid duplication of work.
