Requirements
- High school diploma with relevant community experience or a Bachelor’s degree in a related field.
- Minimum of 2 years of experience in healthcare, public health, social services, or community-based outreach work.
- Valid driver’s license required for community-based fieldwork.
- Strong understanding of healthcare systems and social determinants of health.
- Excellent interpersonal and communication skills.
- Strong organizational and multitasking abilities.
- Proficiency in Microsoft Office tools.
- Experience with data entry and tracking systems.
- Cultural competence and awareness of social determinants influencing health outcomes.
- Fluency in Haitian Creole or Spanish (strongly preferred).
Responsibilities
- Initiate direct outreach to patients in various settings to introduce services and establish engagement.
- Develop, implement, and monitor individualized care plans in collaboration with interdisciplinary teams.
- Provide ongoing education to patients to support disease self-management and health literacy.
- Coordinate with healthcare providers and community organizations to connect patients with essential resources.
- Assist patients in navigating healthcare systems and accessing medication.
- Maintain accurate documentation of patient interactions and care plans using systems like Epic.
- Advocate for patients to remove barriers to care including housing, financial, and behavioral health support.
