Requirements

  • Master's degree in Social Work (MSW) or its equivalent
  • Experience working with pediatric and adolescent populations, including families
  • Experience working in intensive outpatient programs or high acuity behavioral health settings
  • Proficiency with computer and live video tools (e.g., Zoom, Google Meets, DoxyMe, Skype)
  • Skilled in case management, care coordination, and collaborative care delivery
  • Ability to work full-time, approximately 40 hours per week
  • Full-time resident of the United States
  • Successful completion of drug screen and background check

Responsibilities

  • Conduct clinical outreach to families to provide education about VIOP services
  • Screen and assess prospective members for eligibility in the VIOP
  • Guide families through the admissions process, addressing questions about programming and insurance
  • Orient members and their families to the program, technology, and expectations
  • Coordinate care schedules including group, individual, family, and psychiatric services
  • Serve as the primary point of contact for members and families
  • Maintain accurate and timely documentation in the electronic health record
  • Collaborate with multidisciplinary care teams to review progress and develop individualized care plans
  • Conduct comprehensive biopsychosocial assessments
  • Assist with discharge planning and transition to lower levels of care
