Requirements
- Active, unrestricted Nurse Practitioner license.
- Willingness to obtain additional state licenses as needed.
- At least 3 years of Nurse Practitioner experience.
- At least 2 years of experience in adult oncology with urgent care responsibilities.
- At least 1 year of telehealth experience.
- Experience with video conferencing, Google Suite, Slack, and electronic health records (EHR).
- Ability to work effectively under pressure while maintaining accurate documentation.
- Strong listening and communication skills to build rapport with members and colleagues.
- Ability to thrive in a fast-paced, ambiguous startup environment.
- Capacity to prioritize member needs and act decisively.
Responsibilities
- Evaluate and manage members with urgent symptoms, treatment complications, or medical condition exacerbations via a telemedicine platform.
- Assess symptoms, develop comprehensive care plans, triage to appropriate care levels, and educate patients.
- Collaborate with onboarding RNs to address clinical and non-clinical needs, prioritizing goals for Oncology Nurse Navigators and Community Health Workers.
- Review medical records, consult with external providers, and provide clinical insights to the care team.
- Lead interdisciplinary case conferences to create and monitor individualized member care plans.
- Provide clinical oversight to the case management team and perform annual clinical intake visits.
