Requirements
- Doctor of Pharmacy (PharmD) from an accredited institution.
- Licensed Pharmacist in good standing.
- Bilingual fluency in English and either Spanish, Mandarin, or Cantonese.
- Knowledge of CMS quality measures, Star Ratings, HEDIS metrics, and Medicare Part C and D.
- Proficiency in chronic disease state management including polypharmacy.
- Ability to perform medication therapy reviews and apply motivational interviewing techniques.
- Demonstrated reliability, accountability, and strong documentation accuracy.
- Comfort working in a fast-paced, metrics-driven call center environment.
- Strong computer skills including Google Workspace, Slack, and Five9.
- Ability to work 40 hours per week with potential rotating Saturday shifts.
- Must have an established private work area and reliable high-speed internet.
Responsibilities
- Engage patients over the phone to deliver clinical services including MTM, CMRs, TMRs, and medication reconciliation.
- Review medication counseling performed by other virtual pharmacist users.
- Evaluate, assess, and document patient data within the software platform.
- Resolve patient issues through follow-ups with clinics, providers, and pharmacies.
- Develop intervention strategies to optimize medication utilization and meet quality measures.
- Maintain consistent volume of interactions while meeting performance metrics.
- Collaborate cross-functionally to improve platform workflows and automation.
- Provide daily sign-out and handoff communication to ensure continuity of care.
- Coach and support clinical team members and orient new users to the platform.
