Job Details

Experience: Minimum of 1 year of clinical experience working with pregnant and/or postpartum and/or peri/menopause patients is strongly preferred

Requirements

Active independent license in Kentucky: LCSW, LMFT, or LPCC.
Ability to practice independently / without supervision.
Formal perinatal/peri/menopause mental health training or 1+ year clinical experience in the field preferred.
Proficiency with tele-therapy platforms and virtual care delivery.
Perinatal Mental Health Certification (PMH-C) recommended.
Certification in the treatment of the Peri/Menopause population recommended.

Responsibilities

Provide individual tele-therapy to patients requesting regular sessions using FamilyWell’s virtual platform.
Conduct one-on-one psychological intakes for referred FamilyWell program patients, gathering comprehensive assessments to inform treatment planning.
Document patient progress comprehensively, sharing treatment recommendations with medical providers, psychiatric consultants, and other treating professionals as needed.
Facilitate treatment plan modifications for patients needing more intensive or specialized mental health care, ensuring timely and appropriate interventions.
Develop and deliver Care Plans post-session, offering patients actionable insights and follow-up steps.
