Requirements
- Active, unencumbered RN license with multistate privilege (NLC).
- Minimum 3 years of RN experience.
- Direct experience in RPM, CCM, or APCM.
- Working knowledge of EHRs and patient-monitoring software.
- Strong computer and typing skills.
- Familiarity with Medicare care-management documentation and billing.
- Exceptional listening and empathy skills.
- Ability to work independently under general supervision.
- Highly organized and detail-oriented.
Responsibilities
- Log into the Claris Companion platform daily to monitor RPM data and clinical alerts.
- Triage out-of-range readings and reach out promptly to patients.
- Develop and maintain patient-centered, comprehensive care plans.
- Conduct regular patient check-ins and perform medication reconciliation.
- Coordinate care transitions and social-service referrals.
- Document every encounter accurately and log service times for billing.
- Enter billing and diagnosis codes for services provided.
- Maintain audit-ready records and follow HIPAA/compliance standards.
- Collaborate with supervising practitioners and clinical teams.
