Requirements
- Active, unrestricted LPN/LVN license in good standing.
- 2+ years of patient- or community-facing support experience (e.g., care coordination, case management, health coaching, benefits navigation).
- Working knowledge of the U.S. healthcare system and Medicare.
- Ability to work independently, manage a caseload, and maintain strong documentation.
- Reliable computer, internet access, and a quiet space for patient calls.
- Strong communication skills and ability to build trust over the phone.
- Proficiency with technology including EHRs, CRMs, and messaging/scheduling platforms.
- Experience supporting older adults or patients with complex/serious illness (preferred).
- Familiarity with Medicaid and other public programs (preferred).
- Bilingual skills in Spanish or other languages common among Medicare patients (preferred).
Responsibilities
- Serve as the primary point of contact for patients after their intake visit.
- Help patients understand and act on their care plan by scheduling appointments, finding in-network providers, and using Medicare benefits.
- Resolve navigation friction related to portals, referrals, appointment logistics, forms, and provider communications.
- Identify practical and social-needs support needs including food, housing, and transportation to connect patients to resources.
- Coordinate with clinical team members while providing non-clinical support.
- Document all patient interactions and support activities in the platform accurately and on time.
- Maintain confidentiality and comply with HIPAA and privacy standards.
