Requirements

  • Active, unrestricted LPN/LVN license in good standing.
  • 2+ years of patient- or community-facing support experience (e.g., care coordination, case management, health coaching, benefits navigation).
  • Working knowledge of the U.S. healthcare system and Medicare.
  • Ability to work independently, manage a caseload, and maintain strong documentation.
  • Reliable computer, internet access, and a quiet space for patient calls.
  • Strong communication skills and ability to build trust over the phone.
  • Proficiency with technology including EHRs, CRMs, and messaging/scheduling platforms.
  • Experience supporting older adults or patients with complex/serious illness (preferred).
  • Familiarity with Medicaid and other public programs (preferred).
  • Bilingual skills in Spanish or other languages common among Medicare patients (preferred).

Responsibilities

  • Serve as the primary point of contact for patients after their intake visit.
  • Help patients understand and act on their care plan by scheduling appointments, finding in-network providers, and using Medicare benefits.
  • Resolve navigation friction related to portals, referrals, appointment logistics, forms, and provider communications.
  • Identify practical and social-needs support needs including food, housing, and transportation to connect patients to resources.
  • Coordinate with clinical team members while providing non-clinical support.
  • Document all patient interactions and support activities in the platform accurately and on time.
  • Maintain confidentiality and comply with HIPAA and privacy standards.
