- At least 5 years of clinical experience as a Licensed Practical Nurse
Requirements
- Active compact multi-state LPN/LVN license in state of residence.
- Active CA state LVN license.
- 5+ years of clinical experience as an LPN.
- Remote work readiness (stable high-speed WiFi).
- Experience with AI-assisted clinical tools (e.g., Gemini, Open Evidence).
- Experience providing patient education and support for chronic conditions (preferred).
Responsibilities
- Conduct outbound telephonic patient outreach and one-on-one coaching.
- Help patients execute personalized care plans.
- Conduct comprehensive assessments of health status and lifestyle.
- Educate patients on disease management, medication adherence, and symptom recognition.
- Monitor progress and adherence through regular check-ins.
- Document patient interactions and outcomes in the platform.
- Collaborate with clinicians and support teams in real time.
