Requirements
- Active compact multi-state LPN/LVN license in the state where you reside.
- Active CA state LVN license.
- At least 5 years of clinical experience as a Licensed Practical Nurse.
- Remote work readiness with stable high-speed WiFi.
- Experience using AI-assisted clinical tools (e.g., Gemini, Open Evidence).
- Skilled in patient education, coaching, and care coordination.
- Proficiency with Google Workspace and Slack.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions.
- Help patients execute personalized care plans focusing on behavior modification and self-management.
- Conduct comprehensive assessments of health status, lifestyle, and readiness to change.
- Educate patients on medication adherence, symptom recognition, and prevention strategies.
- Monitor progress and health outcomes through remote monitoring tools.
- Document interactions, outcomes, and follow-up plans in the Cadence platform.
- Collaborate with clinicians to identify barriers to care and coordinate next steps.
