Job Details

Experience: At least 5 years of clinical experience as a Licensed Practical Nurse

Requirements

Active compact multi-state LPN/LVN license in the state where you currently reside
Active CA state LVN license
At least 5 years of clinical experience as a Licensed Practical Nurse
Remote work readiness including stable high-speed WiFi
Fluency with AI-assisted clinical tools such as Gemini or Open Evidence
Experience providing patient education and support for chronic conditions

Responsibilities

Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions
Help patients execute personalized care plans and achieve goals using evidence-based techniques
Conduct comprehensive assessments of health status, lifestyle behaviors, and readiness to change
Educate patients on disease management, medication adherence, and symptom recognition
Monitor patients' progress, adherence to treatment plans, and health outcomes
Document patient interactions and outcomes in the Cadence platform
Identify and address barriers to care including social needs
Collaborate in real time with clinicians and support teams using provided tools
