Requirements
- 2+ years in care navigation, case management, or community health.
- Direct experience with SDOH navigation.
- Working knowledge of community resources in PA, NJ, NY, and/or DE.
- Proficient in EMR or care management platforms.
- Strong written communication skills.
- Ability to manage a caseload independently.
- High school diploma or GED required; degree in social work, public health, or human services preferred.
Responsibilities
- Manage an active caseload of patients developing and maintaining individualized care plans.
- Reach patients by SMS and phone to assess needs and clarify barriers.
- Research targeted resources based on patient situation.
- Follow cases through to confirmed resolution.
- Maintain knowledge of resources and eligibility rules across PA, NJ, NY, and DE.
- Navigate public benefit systems including Medicaid, SNAP, and housing programs.
- Document all work in the EMR.
- Collaborate with providers through shared patient records.
