Requirements
- Current, valid, and in good standing New York LPN/LVN Nursing License.
- 3+ years of experience in cardiology, primary care, home care, or chronic care management.
- High level of computer literacy and comfort with digital health portals.
- Ability to work independently and manage time effectively.
- Strong organizational skills and attention to detail.
- HIPAA-compliant, distraction-free work environment.
- Reliable internet connection (minimum 100mbps download / 10mbps upload).
- Current Multistate/Compact Nursing License (preferred).
- Experience in remote monitoring or virtual care management programs (preferred).
Responsibilities
- Conduct virtual patient consultations focusing on education regarding lifestyle and behavior.
- Review and respond to remotely transmitted patient data such as blood pressure, glucose, weight, and pulse oximetry.
- Assist patients in setting up and troubleshooting remote monitoring devices.
- Document all patient communications and activities within patient records.
- Partner with primary and specialty physician care teams to manage patient health outcomes.
