Requirements
- Current, valid, and in good standing Alaska Nursing License (LPN/LVN).
- 3+ years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management.
- Proficiency in using electronic health records (EHR) and care management software.
- Strong communication and interpersonal skills.
- Excellent organizational and time management abilities.
- Ability to work independently and as part of a team.
- Empathy and a patient-centered approach to care.
- Reliable internet connection and HIPAA-compliant work area.
- Proficiency with virtual communication tools such as Zoom and Slack.
Responsibilities
- Complete phone consultations with patients enrolled in care management and/or remote patient monitoring programs.
- Educate patients about their chronic health conditions and lifestyle strategies to manage them effectively.
- Assist patients in setting and reaching goals aligned with provider-approved care plans.
- Maintain accurate and up-to-date patient records per protocol.
- Address patient concerns and barriers to care to improve adherence and outcomes.
- Provide compassionate communication and relay patient needs to their care providers.
- Respond to remotely transmitted patient data, such as blood pressure, glucose, weight, and pulse oximetry.
- Participate in training, team meetings, and quality improvement initiatives.
