Requirements
- Certified Medical Assistant, LPN, or LVN credentials.
- Legally reside and work within the United States.
- High proficiency in computer software and remote technology.
- Strong verbal, phone, and written communication skills.
- Ability to work independently and remotely.
- Dedicated workspace with strict privacy for patient confidentiality.
- High comfort level with fast-paced, high-volume, phone-based outreach.
Responsibilities
- Manage a panel of up to 225 patients providing monthly support.
- Perform structured health assessments.
- Follow and update patient care plans.
- Record all interactions electronically.
- Triage remote monitoring alerts.
- Create electronic alert escalations for physicians/nurses.
- Coordinate care within an extended care team.
- Conduct 30-70 outbound calls daily.
