Requirements
- 3+ years of related work experience in health/nutrition counseling, health coaching, or similar
- Active license in US (preferred)
- Health Coaching or Health Educator certifications (preferred)
- Bilingual, fluent in both English and Spanish
- Skilled in motivational interviewing and driving behavior change
- Comfort and enthusiasm for adopting the latest technologies
- Experience in a high-growth or fast-changing environment
- Excellent interpersonal and communication skills
Responsibilities
- Prioritize and manage individual members along the Twin journey to reversal and self-care management
- Deliver remote coaching via apps, video conferencing, and the Twin Health electronic system
- Engage and activate members to set goals, manage their care, and stay motivated
- Conduct daily monitoring and management of assigned members to ensure program compliance
- Review member health data and reports daily
- Provide feedback to members on metrics like blood glucose, sleep, and nutrition
- Collaborate with providers, nurses, and operations teams to resolve patient care issues
