Requirements
- 3+ years of Nurse Practitioner (NP) experience.
- 2+ years of experience in palliative care.
- Experience caring for patients in outpatient oncology settings.
- Advanced Certified Hospice and Palliative Nurse Practitioner (ACHPN) certification.
- Compact RN license.
- Unrestricted Nurse Practitioner license.
- Willingness to obtain additional state licenses.
- Proficiency in video conferencing, Google Suite, and Slack.
- Experience with electronic health records (EHR).
- Ability to work in a fast-paced, startup environment.
Responsibilities
- Perform specialty palliative care outpatient appointments via video visits.
- Lead interdisciplinary case conferences for members with advanced serious illness.
- Create and monitor individualized care plans for members.
- Provide clinical oversight to the Community Health Worker and Oncology Nurse care management team.
- Educate care team staff on outpatient management of cancer-related symptoms.
- Train staff on serious illness communication skills and advance care planning.
- Develop and implement an advance care planning program.
- Document interactions according to clinical standards.
