Requirements
- Active, unrestricted compact Registered Nurse (RN) license.
- Degree in Nursing (ADN or BSN; BSN preferred).
- Certified Case Manager (CCM) certification.
- Minimum 5 years of nursing experience.
- Minimum 3 years of oncology nursing or case management experience.
- OCN certification required at hire or within 2 years.
- Comfort with video chatting, Google Suite, Slack, and EHR systems.
- Ability to work 11:30 am - 8:00 pm EST shift.
Responsibilities
- Conduct telephonic clinical assessments and medication reconciliation.
- Coordinate discharge plans with hospital case managers and follow-up care.
- Monitor member progress and establish support plans in case conferences.
- Partner with non-clinical staff to support social determinants of health needs.
- Provide referrals for PT, OT, skilled nursing, and hospice care.
- Handle urgent clinical escalations and consult support.
- Document member interactions and communicate feedback to clinical leadership.
