Requirements
- Active, unrestricted compact Registered Nurse (RN) license.
- Degree in Nursing (ADN or BSN required; BSN preferred).
- At least 5 years of nursing experience.
- 3 years of oncology nursing or case management experience.
- Certified Case Manager (CCM) certification.
- OCN certification required at hire or commitment to obtain within 2 years.
- Proficiency with Google Suite, Slack, and electronic health records.
Responsibilities
- Conduct clinical assessments, including pain assessments and medication reconciliation.
- Monitor member progress and establish targeted support plans in case conferences.
- Coordinate care transitions and discharge plans with hospital case managers.
- Provide referrals to PT, OT, skilled nursing, palliative care, and hospice.
- Handle clinical escalations and assist with administrative projects.
- Perform virtual home safety evaluations and assess need for DME.
- Partner with non-clinical team members to address social determinants of health.
