Requirements
- Master's degree in Social Work from a CSWE-accredited program.
- Valid and current master’s level license (LMSW, LISW, LCSW).
- Minimum 3-5 years of experience in a health care setting.
- Experience in oncology, palliative care, serious illness care, or hospice.
- Willingness to obtain additional state licenses as needed.
- Experience with video chatting, Google Suite, Slack, and electronic health records.
Responsibilities
- Conduct comprehensive psychosocial assessments to identify patients' and families' needs.
- Provide individual and family support regarding emotional concerns related to cancer.
- Identify and connect patients and families with relevant community resources.
- Collaborate with oncologists, nurses, and specialists for coordinated care.
- Advocate for patients' needs with insurance companies and healthcare providers.
- Provide crisis intervention support.
- Lead support groups and provide education on cancer-related topics.
