Requirements
- Master's degree in Social Work from a CSWE-accredited program.
- Valid and current master’s level license in current practice state (e.g., LISW, LCSW).
- Minimum 3-5 years of experience in a health care setting.
- Oncology, palliative care, serious illness care, or hospice experience.
- Unrestricted license and willingness to obtain additional state licenses.
- Comfort with video chatting, Google Suite, Slack, and electronic health records.
Responsibilities
- Conduct comprehensive psychosocial assessments to address emotional concerns and social needs.
- Assess, diagnose, and provide therapeutic interventions for behavioral health needs.
- Provide immediate support and intervention in behavioral health crises.
- Identify and connect patients and families with relevant community resources.
- Collaborate with the healthcare team to ensure coordinated care.
- Advocate for patients' needs with insurance and healthcare providers.
- Lead support groups and provide education on cancer-related topics.
