Requirements
- Active, unrestricted compact Registered Nurse (RN) license.
- Degree in Nursing (ADN or BSN); BSN preferred.
- Compact + single state licensure in New York and/or Illinois preferred.
- At least 5 years of nursing experience.
- At least 3 years of solid tumor oncology nursing experience.
- Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) certification required or obtained within 2 years of hire.
Responsibilities
- Conduct comprehensive clinical assessments and oversee member health.
- Facilitate end-of-life care discussions.
- Engage with members via phone and email to provide triage, support, and education.
- Manage symptom management and care coordination.
- Collaborate with Nurse leaders and Medical Directors on clinical protocols.
- Support members throughout the oncology care continuum from screening to survivorship.
- Participate in case conferences to monitor member progress and collaborate on support plans.
- Foster partnerships with payers and providers to optimize care delivery.
