- Experience
- 5 years of nursing experience, including 3 years in outpatient palliative care and/or home hospice
Requirements
- Active, unrestricted compact Registered Nurse (RN) license.
- Degree in Nursing (ADN or BSN; BSN preferred).
- 5+ years of nursing experience.
- 3+ years of experience in outpatient palliative care and/or home hospice with oncology patients.
- OCN certification required at time of hire or commitment to obtain within 2 years.
- Experience managing oncology patients and their caregivers.
- Effective listening and communication skills.
- Multitasking and organizational abilities in fast-paced environments.
Responsibilities
- Conduct comprehensive symptom assessments and support members with serious illness.
- Facilitate end-of-life care discussions when appropriate.
- Manage clinical issues and escalations.
- Collaborate with Nurse leaders and Medical Directors on clinical protocols.
- Establish relationships with members and their care networks via phone, email, and video.
- Coordinate care and provide clinical consult support.
- Collaborate with non-clinical team members on social determinants of health.
- Participate in case conferences and monitor member progress.
