Requirements
- Active, unrestricted compact Registered Nurse (RN) license.
- Degree in Nursing (ADN or BSN).
- Minimum 5 years of professional nursing experience.
- Minimum 3 years of experience in solid tumor oncology nursing.
- Oncology-related certification (OCN, AOCN, AOCNS, or CCM) or agreement to obtain within 2 years.
- Ability to work in the lower 48 United States.
- Strong communication, active listening, and organizational skills.
- Comfortable working in a fast-paced, tech-enabled environment.
Responsibilities
- Conduct comprehensive clinical assessments and oversee member health.
- Provide triage, support, and education to members throughout their cancer journey via phone and email.
- Facilitate end-of-life care discussions and manage urgent clinical escalations.
- Coordinate care across the oncology continuum, from screening to survivorship or end-of-life care.
- Address member needs including symptom management, nutritional support, discharge planning, and provider referrals.
- Collaborate with medical directors and nurse leaders to ensure clinical protocol alignment.
- Partner with payers, oncology providers, and primary care providers to optimize care delivery.
- Work with non-clinical staff to address social determinants of health such as food and transportation.
- Participate in case conferences to monitor and plan member care.
