Requirements
- Active, unrestricted compact Registered Nurse (RN) license.
- Degree in Nursing (ADN or BSN; BSN preferred).
- At least 5 years of nursing experience.
- At least 3 years of high-acuity, adult oncology experience.
- Certified as an OCN, AOCN, AOCNS, or CCM.
- Experience with electronic health records (EHR).
- Experience with video chatting, Google Suite, and Slack.
- Willingness to obtain additional state licenses as needed.
- Must reside within the lower 48 United States.
Responsibilities
- Conduct telephonic clinical assessments including pain assessments and medication reconciliation.
- Monitor member health and coordinate care transitions.
- Educate members and caregivers about diagnosis and treatment plans.
- Identify and prioritize member needs to maintain community safety.
- Assist with care coordination and management following hospital admissions.
- Coordinate discharge plans with case managers and follow up with providers.
- Partner with the care team to address social determinants of health.
- Perform virtual home safety evaluations and assess equipment needs.
- Manage urgent clinical escalations and consults.
