Requirements
- Active, unrestricted compact Registered Nurse (RN) license.
- Degree in Nursing (ADN or BSN); BSN preferred.
- Minimum 5 years of nursing experience.
- Minimum 3 years of high-acuity, adult oncology experience.
- Oncology Certified Nurse (OCN), AOCN, AOCNS, or Certified Case Manager (CCM) certification.
- Experience with remote work environments.
- Proficiency with video chat, Google Suite, and electronic health records.
- Ability to obtain additional state nursing licenses as required.
Responsibilities
- Conduct telephonic clinical assessments and medication reconciliation.
- Monitor member health status and address clinical escalations.
- Coordinate discharge plans with hospital case managers.
- Develop and maintain targeted support plans for oncology members.
- Partner with clinical and non-clinical teams to address social determinants of health.
- Perform virtual home safety evaluations and assess equipment needs.
- Provide referrals to specialized care including PT, OT, and hospice.
