Requirements

Doctor of Pharmacy (PharmD)
Licensed pharmacist in Washington
Completion of a PGY2 Oncology Residency OR PGY1 Ambulatory Care Residency and at least 3 years of experience as a clinical oncology pharmacist OR at least 3 years of experience as a clinical pharmacist in oncology
1 year practicing under a Collaborative Practice Agreement (CPA)
Strong communication, organization, and tech skills
Empathy, integrity, and a team-first mindset

Responsibilities

Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‑level assessments.
Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA.
Promote seamless medication coordination.
Monitor therapy effectiveness and safety, identify optimization and intervene as appropriate.
Support quality‑improvement and REMS initiatives.
Provide recommendations to operations by supporting teammates with complex prior authorizations.
Improve workflows and educate patients.
Maintain ongoing clinical development.
