Job Details

Experience: Completion of a PGY2 Ambulatory Care Residency OR Completion of a PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA) OR At least 3 years of ambulatory practice with disease state management

Requirements

Doctor of Pharmacy (PharmD)
Licensed pharmacist in California
Completion of a PGY2 Ambulatory Care Residency, OR completion of a PGY1 Residency and at least 1 year of ambulatory practice under a CPA, OR at least 3 years of ambulatory practice with disease state management
Strong communication, organization, and tech skills
Empathy, integrity, and a team-first mindset

Responsibilities

Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‑level assessments.
Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA.
Promote seamless medication coordination.
Monitor therapy effectiveness and safety.
Identify optimization opportunities and intervene as appropriate.
Support quality‑improvement and REMS initiatives.
Provide recommendations to operations by supporting teammates with complex prior authorizations.
Improve workflows and educate patients.
