Requirements
- Active, unrestricted Registered Nurse (RN) license in a compact state.
- Ability to obtain additional state licenses upon hire.
- Minimum 2–3 years of clinical nursing experience.
- Experience in a telehealth or virtual care setting.
- Experience in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), or care management.
- Proficiency with EHR systems and telehealth platforms.
- Strong clinical assessment and care coordination skills.
- Excellent verbal and written communication skills.
- Dedicated, private home workspace.
- Windows 10 or MacOS 13 computer with stable internet (50 Mbps down/20 Mbps up) via ethernet connection.
Responsibilities
- Prepare comprehensive AWV charts supporting provider efficiency and CMS compliance.
- Conduct virtual AWV reviews covering history, medication adherence, and preventive screenings.
- Identify and address patient care gaps and risk factors.
- Provide telephonic and portal-based care management for complex patients.
- Create and monitor individualized care plans for CCM, RPM, RTM, and APCM programs.
- Utilize motivational interviewing to engage patients in care planning.
- Communicate patient status updates to primary care providers and interdisciplinary teams.
- Coordinate orders, referrals, and follow-up actions to ensure care continuity.
- Maintain accurate patient records in electronic health record systems (CT360, RXNT, Zoom).
