Responsibilities
- Educate potential patients and family members on the program and program benefits
- Assist patients in navigating the program app and website
- Initiate outbound calls and respond to inbound calls promptly
- Assess patient eligibility and address patient inquiries
- Maintain accurate records and call notes
- Achieve weekly productivity targets
- Ensure timely follow-up for interested patients
- Send educational materials via email and communication channels
