Requirements
- Bachelor of Social Work (BSW), Associate Degree in Nursing (ADN), Doula certification, or education background preferred
- 3+ years of experience in a virtual patient-facing role
- Experience in patient engagement, care coordination, social services, or healthcare services
- Experience working with pediatric populations or young families preferred
- Proficiency with Electronic Health Records (EHR) and technology troubleshooting
- Excellent verbal and written communication skills
- Strong organizational skills and attention to detail
- Ability to work independently and collaboratively in interdisciplinary teams
Responsibilities
- Conduct proactive outreach and scheduling via phone and secure messaging
- Educate patients on services and assist with portal/telehealth technology setup
- Facilitate care coordination and communication between patients, clinical staff, and providers
- Identify social determinants of health and refer patients to community resources
- Maintain accurate documentation of interactions in EHR systems
- Ensure HIPAA compliance in all communications and record handling
- Participate in weekly registry review sessions and document coordination needs
