Requirements

  • Master’s degree in social work, counseling, public health or related field.
  • Valid LCSW, LPC, LMFT credential or valid QMHP credential.
  • Minimum 5 years’ related experience (e.g., Case Manager, Care Coordinator, Quality Improvement Specialist).
  • 2 years’ experience in behavioral health outcomes-based care.
  • 2 years’ experience designing and delivering training/technical assistance to professional audiences.
  • Experience in the Medicaid environment.
  • Applied knowledge in clinic operations and charting systems (EHR).
  • Applied knowledge with quality improvement tools and techniques.
  • Applied knowledge in Adult Learning Theory and training design.
  • Advanced computer application skills (MS Office, Visio, etc.).

Responsibilities

  • Research approaches to outcomes-based care and market practices.
  • Design and implement projects relating to outcomes-based care in behavioral health networks.
  • Monitor and communicate project status, timelines, and budget progress.
  • Serve as primary liaison with web-based outcomes application vendors.
  • Analyze and communicate complex clinical data for internal and external parties.
  • Provide technical assistance and support to clinical/operational leadership.
  • Develop and deliver training resources through a train-the-trainer program.
  • Assist providers with planning and reporting for outcomes-based care payments.
