Requirements

Graduate of an Accredited School of Nursing
Current unencumbered RN license
Nursing knowledge and skills for treating frail, elderly participants
Ability to manage complex clinical situations
Highly motivated and self-directed
Ability to execute tasks in a quickly changing environment
Ability to make sound decisions in emergency situations
Excellent clinical, organizational, and communication skills
Must be able to work every other weekend
Triaging experience (preferred)

Responsibilities

Answer phones after hours including evenings, nights, weekends, and holidays
Resolve logistical issues over the phone and during home/acute care visits
Handle inbound and outbound calls within scope of practice
Coordinate telehealth meetings between participants and clinicians
Troubleshoot logistical issues such as transportation, medication delivery, and hospital discharge
Support clinical care teams with smooth care transitions between settings
Escalate changes in participant condition as appropriate
