Requirements
- Graduate of an Accredited School of Nursing
- Current unencumbered RN license
- Nursing knowledge and skills for frail, elderly participants
- Ability to manage complex clinical situations
- Highly motivated and self-directed
- Ability to make sound decisions in emergency situations
- Excellent clinical, organizational, and communication skills
- Ability to work assigned shifts including days, evenings, nights, weekends, holidays, and overtime
Responsibilities
- Answer phones after hours including evenings, nights, weekends, and holidays.
- Resolve logistical issues for participants via phone and in-person visits.
- Consult with other clinical staff on-call as needed.
- Handle inbound and outbound calls within scope of practice.
- Coordinate telehealth meetings between participants and clinicians.
- Troubleshoot and resolve logistical care delivery issues such as transportation, medication delivery, and hospital discharge.
- Support clinical care and home health teams with transitions between care settings.
