- 2+ years of experience in primary care, telehealth, or care management.
- Familiarity with Medicare programs.
- Knowledge of care navigation and incident-to requirements.
- Understanding of SDOH documentation.
- Proficiency with EMRs, care management platforms, and telehealth workflows.
Responsibilities
- Conduct initiating visits and telehealth-based E/M or Annual Wellness Visits.
- Establish medical necessity for care navigation services.
- Identify high-risk conditions and unmet social needs.
- Document findings in the platform using accurate coding, SDOH Z-codes, and risk factors.
- Validate and develop individualized care plans.
- Collaborate with advocates and nurse care managers to finalize care plans.
