Requirements
- Master's degree as a Nurse Practitioner
- Board certification (ANCC or AANP)
- Active NP license
- Multi-state compact RN licensure
- 5+ years of NP experience treating chronic conditions (heart failure, type 2 diabetes, hypertension)
- 5+ years leading clinical teams of NPs
- Willingness to obtain additional state licensure as needed
- Experience with remote patient monitoring technology is a plus
Responsibilities
- Manage patients through virtual encounters, reviewing vital signs, laboratory results, and symptoms in coordination with Clinical Navigators and RNs to drive timely, guideline-concordant treatment adjustments
- Initiate and optimize guideline-directed medical therapy for heart failure, type 2 diabetes, and hypertension
- Conduct monthly clinical audits, monitor team performance data for trends, and implement quality improvement strategies
- Coach, mentor, and develop a team of Nurse Practitioners through regular feedback and clinical education
- Oversee team operations including scheduling, PTO and CME approval, and staffing coverage
- Translate updated clinical guidelines and organizational policies into team practice
