Completion of an accredited Nurse Practitioner program.
Active Nurse Practitioner or APRN license in good standing.
Licensed in multiple states.
2+ years of experience in primary care, telehealth, or care management.
Familiarity with Medicare programs.
Experience with care navigation and incident-to requirements.
Proficiency in SDOH documentation.
Proficiency with EMRs and care management platforms.
Experience with telehealth workflows.

Responsibilities

Conduct initiating telehealth visits for E/M or Annual Wellness Visits.
Establish medical necessity for care navigation programs.
Identify high-risk conditions and unmet social needs.
Document findings in platform using accurate coding, SDOH Z-codes, and diagnoses.
Develop and approve individualized care plans.
Collaborate with care advocates and nurse care managers to finalize care plans.
