Requirements

  • 3+ years of experience in healthcare, social services, or advocacy.
  • Strong working knowledge of Medicare and the U.S. healthcare system.
  • Ability to manage activities in a self-directed, independent manner.
  • Comfort using technology platforms, EHRs, CRMs, or scheduling tools.
  • Experience supporting older adults or populations with complex needs.
  • Strong problem-solving and interpersonal communication skills.
  • Commitment to member empowerment and reducing barriers to care.
  • Relevant professional credentials (e.g., RN, CMA, BCPA) are preferred.

Responsibilities

  • Serve as a member-facing contact for Medicare patients after intake appointments.
  • Help members navigate healthcare services, insurance, and appointment scheduling.
  • Assist with navigation issues like online portals, referrals, and application forms.
  • Identify Social Determinants of Health support needs and available resources.
  • Coordinate with clinical professionals for non-clinical navigation.
  • Document member interactions and support activities accurately.
  • Provide empathetic and culturally responsive member support.
  • Exercise independent judgment in performing navigation services.
