Requirements

At least 2 years of experience as a front desk receptionist
At least 2 years of experience as patient coordinator
At least 2 years of experience in a similar role within a U.S.-based healthcare practice
Experience with Chronic Care Management (CCM) strongly preferred
Bilingual in English and Spanish
Excellent written communication skills
Excellent verbal communication skills
Comfortable being on camera throughout the full 8-hour shift

Responsibilities

Conduct monthly outbound calls to Medicare patients enrolled in Chronic Care Management programs
Follow up with patients to monitor health status, address concerns, and ensure continuity of care
Schedule appointments and coordinate follow-ups as needed
Maintain accurate and timely documentation of all patient interactions
Support patient adherence to care plans through consistent communication
Collaborate with healthcare providers and administrative teams to ensure high-quality patient care
