Requirements:
Master's degree in Social Work (MSW) and Associate Registration (or state equivalent) Experience working in intensive outpatient programs or high acuity behavioral health settings within a multidisciplinary team preferred Experience working with pediatric and adolescent populations, including families Computer and live video tools literacy (e.g., Zoom, Google Meets, DoxyMe, Skype, etc.) Skilled in case management, care coordination, and collaborative care delivery Organized and excited by a dynamic environment Passionate about helping kids and teens achieve goals Ability to work full-time, approximately 40 hours per week Candidates must successfully pass a drug screen and background check prior to employment Full-time resident of the United States Additional language skills are beneficial
Responsibilities:
Conduct clinical outreach to families about VIOP services Screen and assess prospective members for VIOP eligibility Guide families through admissions process Ensure timely scheduling of intake assessments Orient members and families to the VIOP program Coordinate care schedules including therapy and psychiatric services Serve as primary point of contact for members and families Coordinate external case management needs with schools, PCPs, and community resources Assist with discharge planning Maintain accurate documentation in EHR Engage in multidisciplinary care team staffing meetings Act as liaison between families, VIOP team, and external providers Collaborate with internal teams to address barriers to care Conduct comprehensive biopsychosocial assessments Develop individualized care plans Support group facilitators by managing member needs Provide clinical session coverage as needed
