Outpatient Program Specialist

Bachelor’s degree in nursing, public administration, public policy, public health, or related field Proven ability to design, implement, and support a regional technical assistance strategy Comprehensive knowledge of primary care delivery, chronic disease management processes, and data reporting requirements In-depth understanding of clinical quality measures and QI methodologies Proven ability to design and deliver effective training programs, workshops, and learning collaboratives

Oversee quality improvement initiatives (chronic disease management, behavioral health, data reporting) Support Quality Improvement Advisors (QIAs) Serve as a subject matter expert in outpatient primary care practice areas Engage with state, regional, and national partners
