Director of Population Health

5+ years in population health, medical economics, managed care, or healthcare analytics. Strong understanding of claims data, cost/utilization modeling, and quality measurement frameworks (e.g., HEDIS, Stars, MLR). Proven ability to translate data insights into operational programs. Experience partnering with clinicians and care teams to drive performance. Excellent communicator who can distill complexity into clarity for diverse stakeholders. Passion for transforming cardiovascular outcomes through technology and systems thinking.

Lead the development of population health analytics and dashboards. Conduct analyses to pinpoint care gaps, high-cost drivers, and variation. Translate complex data into actionable strategies. Partner with Finance and the Medical Director to model ROI for interventions. Work closely with the Medical Director to design and implement programs. Translate payer and risk contract goals into workflows. Define and track KPIs for program execution. Partner with Product and Data teams to enhance tools. Serve as a bridge between payer analytics and clinical. Collaborate with leadership to prioritize interventions. Prepare performance narratives for reporting.
