Bachelor’s degree in Social Work from an accredited college or university OR Minimum two (2) years of experience as a case manager with a CCM (Certification in Case Management) in a community-based and/or medical setting with an understanding of behavioral health prevention science Basic Life Support (BLS) certification from the American Heart Association or American Red Cross Master’s degree in Social Work (preferred) Experience in substance abuse screening, use, and/or treatment (preferred) Experience with screening, brief intervention, and referral to treatment (SBIRT) (preferred) Experience with motivational interviewing (MI) and smoking cessation services (preferred) Experience working in behavioral health and/or human services (preferred) Proficiency with Electronic Medical Record (EMR) system, as well as computer and web-based interfaces (preferred) Bilingual English/Spanish language skills (preferred)
