- Doctor of Pharmacy (PharmD) or equivalent advanced degree
- Active pharmacist license in good standing in Illinois and at least one additional U.S. state
- Must reside and be authorized to work in a U.S. state
- Completion of an accredited residency program (PGY-1 or PGY-2)
