Requirements:
Doctor of Pharmacy (PharmD) or equivalent advanced degree required. Licensed Pharmacist required. Massachusetts (MA) License required. 1+ years' experience is strongly preferred. Ability to obtain additional licensure within a mutually agreed upon timeframe if needed. Strong verbal and written communication skills. Deep knowledge of specialty medication pharmacology. Knowledge of local/state/federal pharmacy regulations. Able to manage processes. Strong technical and clinical skills. Excellent leader and manager of people. Constant focus on legal compliance. Highly productive, quality focused, and attention to detail. Computer literate with proficiency in database and reporting tools such as Microsoft Word, Excel, and PowerPoint. Knowledge using EPIC/QS1/WILLOW software is a plus. Ability to identify barriers to medication adherence and implement and monitor plans to improve adherence. Ability to motivate and coach patients to achieve better outcomes and identify goals of therapy. Organized, high-integrity, dependable, empathetic, good listener/communicator. Energetic, highly motivated, team player. Discretion and confidentiality essential. Knowledge of complex/chronic diseases states and therapies within specialty pharmacy (Oncology, Transplant, Hepatitis C, Infectious Disease (including HIV), Multiple Sclerosis, Rheumatoid Arthritis, GI disorders (including IBD), Cystic Fibrosis, Psoriasis, Hyperlipidemia and inflammatory conditions (including Lupus)) preferred.
Responsibilities:
Review and interpret clinician's patient care notes for prior authorization approvals and care plan recommendations. Develop and refine disease and drug specific clinical protocols. Monitor patient adherence across all sites and disease categories. Evaluate EMR for drug/disease interactions, medication selection, labs, allergies, and RX orders. Recommend alternative therapeutic drug regimens and treatments according to guidelines. Monitor and evaluate therapies for side effects and recommend drug therapy changes. Perform medication therapy management (MTM) including reviewing regimens and documenting interventions. Conduct clinical interventions such as pharmacist consultations, motivational interviewing, and medication reconciliation. Oversee tech's daily activities for accuracy in delivery coordination, prior authorizations, and proactive management care utilization. Identify and resolve insurance related issues and financial assistance opportunities. Supervise communication of support personnel compliant with HIPAA. Maintain current registration and comply with drug laws. Communicate with other health professionals regarding patient care. Develop own knowledge and train pharmacy staff and healthcare professionals. Manage, organize, and update relevant data using database applications. Apply observations and recommendations to operational issues. Assist with research projects and peer reviewing articles. Identify trends, resolve problems, recommend improvements, and implement change. Actively participate in process improvement initiatives. Keep information confidential. Update job knowledge through educational opportunities, publications, and professional networks. Willing to do whatever is necessary for the betterment of the Company. Other duties as assigned.
